Technology Help Desk Hardware Loan Agreement

Summary

Borrowers of devices from the LITS Technology Helpdesk must follow the acceptable use policy as for any other device and return it as outlined in the loan agreement.

Staff/Faculty Computer/Device Loan Agreement

As a borrower of an MHC device through the Technology Help Desk Computer Loan Program, I have read and acknowledged the following expectations and understand that I am responsible to:

The standard loan period is two weeks, and I can request an extension of the loan period for as long as the need exists by contacting the LITS Technology Help Desk.

If I resign or retire before the end of the loan period, the devices will be returned to Human Resources or directly to LITS. Failure to return the device in these circumstances will result in the device being locked and further actions taken to secure the return of the loan computer.

If I fail to return the device at the end of the loan period, I may not be eligible for a new computer under the computer replacement policy, and LITS may take additional action to secure the return of the loan computer.
